This book has its origins in my PhD thesis written for the University of Sydney, Australia. I wish to express my deepest and most sincere gratitude to Jane Simpson and Derek Herforth, my supervisors, for their remarkable patience and professional guidance. The thesis would not have been possible without their kind support, invaluable advice, probing questions, as well as constructive critiques.
I would also like to express my gratitude to Stephen Matthews, Foongha Yap and Antony Diller, for their very useful comments on my PhD thesis and their encouragement to publish my thesis in the form of a book.
